Process for obtaining a Death certificate?

      A death certificate can be obtained by the next-of-kin of the deceased from any of our State Offices across the country and the FCT office on presentation of the following:

1. Presentation of a notification of the cause of death from the hospital
2. A court Affidavit
3. A means of identification of the declarant (International Passport, Driver’s License, National Identity Card, Voter’s Card)
4. Two (2) Passport Photographs
